Is 'Leaky Gut' Real? What the Research Actually Shows

'Leaky gut' has become a headline term in wellness circles, sometimes used to explain everything from eczema to autoimmune disease to anxiety. The overuse of the term has led some people to dismiss it entirely as a wellness myth. The reality, as ever, is more nuanced.


Intestinal permeability is a legitimate, measurable, and well-researched physiological phenomenon. What's less established is the specific 'leaky gut syndrome' often described in popular health content as the singular cause of every chronic illness. Let me explain what we actually know.


The gut barrier: what it is and what it does

Your gut lining is a single layer of epithelial cells. Despite being just one cell thick, it acts as a highly sophisticated selective barrier- allowing nutrients, water, and certain molecules to pass into the bloodstream while keeping out bacteria, toxins, and incompletely digested food particles.


The integrity of this barrier depends on structures called tight junctions: proteins that link neighbouring epithelial cells together. When tight junctions are compromised, the barrier becomes more permeable. This is what 'leaky gut', more precisely called increased intestinal permeability, actually refers to.


What the research establishes about leaky gut

A 2024 review in Digestive Diseases confirmed that increased intestinal permeability is well-documented in association with several conditions, including:

  • Inflammatory bowel disease (IBD): including Crohn's disease and ulcerative colitis

  • Irritable bowel syndrome (IBS)

  • Type 2 diabetes

  • Non-alcoholic fatty liver disease

  • Autoimmune conditions including Hashimoto's thyroiditis and rheumatoid arthritis

  • Food allergies


A comprehensive 2023 narrative review in Springer (Internal and Emergency Medicine) further confirmed that disruption of the gut barrier allows bacterial metabolites and endotoxins, including lipopolysaccharide (LPS), to enter circulation, triggering low-grade systemic inflammation. This mechanism has real, documented downstream effects.


What's less certain

What's not established is whether increased intestinal permeability is always a cause of these conditions, or whether it's often a consequence, or both. The research is frequently associative rather than causative. It's also not established that every person who feels unwell has 'leaky gut,' or that leaky gut is the primary driver of conditions like anxiety or fatigue in the absence of other clear markers.


Additionally, the specific 'leaky gut protocol' products and supplements marketed online, many of which are sold at significant cost, are often ahead of the evidence. Some ingredients (like glutamine and zinc carnosine) have genuine mechanistic support for gut barrier repair; many others do not.


Zonulin has been widely promoted as the definitive biomarker of intestinal permeability. However, more recent research has raised questions about the specificity of commercial zonulin assays. Gut permeability is best assessed in clinical context alongside symptoms and other markers, not through a single test result in isolation.


What does affect gut barrier integrity?

The research is clear on several factors that negatively impact tight junction integrity and increase permeability:

  • Chronic stress (via cortisol and reduced secretory IgA)

  • Ultra-processed diets high in emulsifiers, refined carbohydrates, and highly processed seed oils

  • Alcohol (a well-established gut barrier disruptor)

  • Certain medications including NSAIDs and antibiotics

  • Dysbiosis (an imbalance in the gut microbiome, favouring less beneficial strains)

  • Inadequate fibre intake, which reduces production of gut-protective butyrate


What supports gut barrier repair?

Food-first strategies with the strongest evidence include:

  • Dietary fibre and prebiotic foods to produce short-chain fatty acids (particularly butyrate) that directly nourish the gut lining

  • Polyphenol-rich foods (berries, olive oil, green tea) which have anti-inflammatory and microbiome-supportive effects

  • Fermented foods (kefir, sauerkraut, kimchi) to support beneficial bacterial populations

  • Zinc-rich foods, as zinc is essential for tight junction protein function

  • Adequate protein, as the gut lining cells are among the fastest-renewing in the body


So, is leaky gut real? Yes. Is it the root cause of everything? No. Is it worth investigating if you have gut symptoms, autoimmune conditions, or chronic skin or inflammatory conditions? Almost certainly.
